After Hours Pediatric Triage on the Rise

Pat Miley, RN was the Nurse Manager of the first Primary Care After Hours Clinic in Ann Arbor, Michigan for 10 years, teaching, training and supervising registered nurses in quality pediatric triage.

Within a short period of time, Pat began triaging pediatric calls through the night from her home for one pediatrician, which rapidly increased to several others. 

As the obvious need for quality after hours pediatric triage increased, Pat recognized the need to form Physicians' Triage Service, P.C. so that many other pediatricians and their patients could benefit from the highest quality of after hour care. 

Physicians' Triage Service understands that treating both the child and the parent is essential in Pediatrics.

The President and owner, Pat Miley, RN, personally selects our nurses for employment.

An Actual Call

One early November morning at 1 am, Physicians' Triage Service received a call about a first grader who had not been feeling well for several hours. The parents were concerned by the child's abnormal behavior during this time. She appeared lethargic, uninterested in fluids and did not respond to touch.

The parents denied any fever, cold symptoms, vomiting, respiratory difficulty, etc. After a bit more probing, it was discovered that the child had a rash, which was described as dark red and purple scattered on her legs. Our nurse made the quick assessment that this was an emergency situation and advised ER evaluation immediately. The parents were not convinced of this urgent matter until the nurse clearly explained the emergent need for immediate ER evaluation. The parents agreed with this plan and left immediately.

Diagnosis: Meningococcemia. The child would have died before morning if not triaged thoroughly.
